Job summary

LifeSpan Therapy in Indiana is seeking a Restorative Certified Nursing Assistant (CNA) to support residents' independence through restorative care and collaboration with the therapy team. This role emphasizes compassionate, person-centered care and aims to enhance mobility and daily living activities.

You will implement individualized care plans, monitor progress, document changes in the clinical record, and respond to residents' needs promptly.

Qualifications

• Current Indiana CNA certification is required.
• Experience in resident care within healthcare or long-term care is preferred.
• Ability to use basic computer applications for documentation and communication.

Responsibilities

• Delivers quality care with kindness and respect for resident dignity.
• Receives and completes patient care assignments from Therapy Supervisor.
• Provides direct care to maintain or improve residents' abilities per care plans.
• Protects residents' rights, privacy, and confidentiality.
• Responds promptly to call lights and emergencies.
• Records accurate, legible resident care information in the clinical record.
• Monitors changes in residents' psychosocial status and reports to Charge Nurse.
• Follows infection control procedures; adheres to facility policies.
