Job summary

Windsor Gardens – Lancaster, TX invites applications for a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members. You will provide personal care to patients safely and promote comfort and independence.

You will assist with ambulation, ROM, positioning, and self-feeding as trained, while documenting observations and reporting condition changes to the DOR.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Must be able to read, write, and speak English.
• Available to work six days per week.
• Ability to travel to additional SNF locations within geographical area.

Responsibilities

• Provide personal care to patients safely and comfortably.
• Assist in restorative and rehabilitative care per care plan.
• Perform restorative tasks such as ambulation, ROM, positioning, feeding as trained.
• Promote independence and self-care for patients.
• Observe, report and document patients' conditions daily.
• Advise Director of Rehab of patient status daily.

Windsor Gardens- Lancaster, TXThe overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP members as appropriate.Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care.Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.Promotes and supports the greatest possible degree of independence for patients.Advises Director of Rehab (DOR) of Patient's status daily.Contributes knowledge of Patient's conditions/needs to patient care plans.Assists in the implementation of care plans as directed within the parameters of established policy and procedure.Observes and reports on Patients' conditions and documents observations as required.State certification as a Nursing Assistant is mandatory.Ability to read, write, and speak the English language.Must be available to work six days per weekAbility to travel to additional SNF locations within geographical areaDiversity, Equity, and Inclusion are at the heart of Pivot.
