Job summary

LiveWell Homecare Agency, LLC seeks a licensed Registered Nurse (RN) in Ohio to deliver holistic, patient-centered home care using the nursing process. You will assess physical, psychosocial, spiritual, and educational needs and develop collaborative care plans with patients and families.

Responsibilities include implementing evidence-based interventions, educating patients, responding to emergencies, and documenting care in the EMR.

Qualifications

Must hold current Ohio RN license and BLS certification.
ACLS required within six months for units that require it.
Ability to provide culturally and age-appropriate care.
Bachelor of Science in Nursing (BSN) within five years of hire.
No prior experience required for select entry roles.

Responsibilities

Conduct comprehensive patient assessments of physical, psychosocial, spiritual, and educational needs.
Develop and communicate individualized care plans with patients, families, and the team.
Implement evidence-based interventions and provide patient education.
Respond to emergencies with skill and accountability.
Evaluate patient responses and adjust care plans to optimize outcomes.
Accurately document care in electronic medical records.
Demonstrate leadership by mentoring peers and supporting quality improvement.
