Job summary

LiveWell Homecare Agency, LLC is seeking an RN to provide holistic, patient-centered nursing care across biological, physical, behavioral, social and cultural domains in Ohio.

You will assess patients, develop and update individualized care plans with families, implement evidence-based interventions, educate, respond to emergencies, document activities, and mentor peers while participating in quality improvement and professional growth.

Qualifications

• RN licensure in Ohio is required.
• BLS certification; ACLS if in ICU/step-down.
• BSN within five years for nurses hired after January 2010.

Responsibilities

• Conduct comprehensive patient assessments to identify needs and safety risks.
• Develop and update individualized care plans in collaboration with patients and families.
• Implement evidence-based interventions, provide education, and respond to emergencies.
• Evaluate care effectiveness and adjust plans to improve outcomes.
• Accurately document all nursing activities and patient responses.
• Provide leadership by mentoring peers and supporting quality improvement and professional growth.
