Job summary

Responsibilities

• Assess patients’ physical, psychosocial, spiritual, and educational needs.
• Identify problems and develop individualized care plans with patients and families.
• Deliver evidence-based care and educate patients; respond to emergencies.
• Monitor, evaluate, and adjust care with continuity during handoffs.
• Accurately document all care activities in EMR.
