Job summary

You will provide personalized navigation and follow-up support to move clients beyond crisis toward long-term self-sufficiency.

Responsibilities include conducting needs assessments, offering information and referrals, assisting with action plans, and following up to monitor progress.

Qualifications

• Experience in human services, social services, case management, customer service, or related field preferred.
• Knowledge of public benefits and community resources preferred.
• Strong organizational and communication skills.
• Ability to work with individuals from diverse backgrounds.
• Proficiency with computers and client management systems.

Responsibilities
