Job summary

Nevada Partnership for Homeless Youth (NPHY) is seeking a Homeless Youth Residential Case Manager to provide case management to ILP participants in a full-time exempt role. Under the Housing Programs Manager, you will stabilize youth upon entry, connect to education and employment resources, advocate for services, and guide graduates into aftercare.

You will collaborate with Direct Care staff and agency partners, maintain HMIS records, and assist youth with housing transitions while ensuring

Qualifications

• Bachelor’s Degree in Social Work is required or preferred.
• LSW license in NV strongly preferred.
• Must be 21+ years old with valid driver's license.

Responsibilities

• Manage caseload of youth in Independent Living Program (ILP).
• Support youth in identifying short- and long-term needs and steps to self-sufficiency.
• Assist with health appointments and referrals to mainstream services.
