Job summary

Coordinated Care Services, Inc. is seeking a Residential Team Lead Habilitator to supervise Direct Support Professionals on shift at the Gorham Residential site. The role ensures quality service delivery and acts as a liaison between DSPs and the supervisor.

The position requires strong leadership, planning and communication skills, with prior experience serving individuals with disabilities preferred. A reliable transportation between locations is expected.

Qualifications

• High school diploma or equivalent required; an associate’s degree in human services preferred.
• Experience serving individuals with disabilities is preferred.
• Ability to plan and organize daily operations and communicate effectively.

Responsibilities

• Imparts agency philosophy of Equal Opportunity, Independence and Realization of Potential to staff and clients.
• Oversees safety and supervision of individuals on shift.
• Completes documentation in a timely manner and maintains reports.
• Trains new team members and provides problem‑solving support to staff.
• Communicates policies, procedures, and goals to employees clearly.
• Maintains billing records per policy and regulatory requirements.
