Job summary

Monarch Healthcare is seeking personnel for residential direct care positions in Robbins, IL. Sleepovers are required to support individuals with mental health needs, developmental disabilities, or substance use disorders.

The role involves assisting with daily living tasks, community integration, safety procedures, and detailed documentation to ensure person-centered plans are implemented effectively.

Qualifications

• Sleepover at a residential setting is required.
• Experience working with individuals with mental health diagnoses, intellectual/developmental disabilities, or substance use disorders (population served).
• Certification: Drivers License (Valid) - USA

Responsibilities

• Assess each person for hopes, dreams and desires and provide supports to facilitate achievement.
• Support individuals in developing relationships in their community and with natural supports.
• Assist individuals in participating fully in the community aligned with their interests.
• Protect and promote the rights of individuals; assist in advocacy and using personal advocates.
• Provide support to meet emotional, physical, and medical needs.
• Foster person-centered, positive relationships and self-determination.
• Provide input into assessment and planning, and develop the individual’s plan.
• Implement the plan and document progress, including goals and incidents.
• Complete daily progress notes and communication logs.
• Attend meetings and trainings; maintain required certifications.
• Comply with agency policies and state/federal regulations.
• Assist with day-to-day home operations (housekeeping, meals, laundry, shopping, etc.).
• Assist during late-night hours when required.
• Provide transportation as needed.
• Ensure knowledge of emergency procedures and safety; conduct drills and inspections.
• Assist staff with orientation and training.
• Follow service definition guidelines and perform other duties as assigned by supervisor.
