Qualifications

• High School Diploma required.
• Valid Driver's License (USA) required.
• Experience with individuals with mental health diagnoses, intellectual/developmental disabilities, or substance use disorders is preferred but not required.

Responsibilities

• Assess each person’s hopes, dreams, and provide supports to facilitate achievement.
• Support people in developing relationships in their community and with their natural supports.
• Assist people in participating fully in their community aligned with their interests.
• Ensure the rights of each person are protected and promoted at all times.
• Assist people in developing advocacy skills and using personal advocates.
• Provide support to meet emotional, physical, and medical needs of each person.
• Foster person-centered, positive relationships promoting self-determination.
• Provide input into assessment and planning, and development of the individual’s plan.
• Implement the plan fully and document requirements (goals, actions, incidents, timesheets).
• Complete daily progress notes and communication logs to assure appointments, goals, and interests are met.
• Attend trainings and maintain certifications as required.
• Comply with agency policies and state/federal regulations relevant to program areas.
• Assist with day-to-day home operations (housekeeping, meals, laundry, shopping, etc.).
• May assist during late night hours as needed.
• Drive and travel as required; arrange transportation as needed.
• Know emergency procedures and safety, conduct drills and inspect equipment.
• Assist new staff with orientation, mentoring, and training.
• Follow service definition guidelines for services provided.
• Complete other duties as assigned by supervisor.
