Job summary

Vinfen seeks a dedicated support worker to assist in developing and documenting individualized treatment plans. You will provide role-modeling for individuals served, support their integration into communities, and monitor changes in their physical and emotional status.

The role involves ensuring site safety, managing daily living tasks, and following MAP policies for medication handling. Driving and certifications may be required per program needs.

Qualifications

• One to three months experience in a human services setting, or equivalent.
• High school diploma or equivalent required.

Responsibilities

• Assist in developing, implementing, and documenting individual treatment support plans.
• Provide role model and support to persons served; help integrate into communities.
• Assess physical and emotional status and report changes to supervisor.
• Ensure site safety and maintenance; assist with daily living tasks and cleaning.
• Store, administer, document and dispose of medication per MAP and policy.
• Complete various paperwork and logs; provide crisis intervention as needed.
