Qualifications

• Experience working with people with disabilities preferred.
• A high school diploma or equivalent is required.
• May require driving and valid license; background checks may be needed.

Responsibilities

• Develop, implement, and document individual treatment support plans.
• Carry out rehabilitative and behavioral interventions and monitor progress.
• Provide role model support for client integration into communities.
• Assess physical and emotional status and report changes to supervisor.
• Maintain site safety and assist with daily living tasks with clients.
• Complete paperwork and logs including logs, schedules, and forms.
• Perform crisis intervention as needed.
