Qualifications

• High school diploma or equivalent required; Bachelor's degree preferred.
• 1-2 years of related experience or training.
• Driving is required with valid license and background check.
• Ability to lift 25 pounds and assist daily living tasks.
• MAP, Safety Care, NET certifications or willingness to obtain.

Responsibilities

• Provide individual and group support in the home.
• Plan recreational activities promoting inclusion and wellbeing.
• Assist with community-based supports and resources access.
• Maintain a safe, clean residential environment and records.
• Administer medications per MAP and document accordingly.
• Support shift operations and lead clinical programming as assigned.
• Train individuals in rehabilitative and behavioral interventions.
• Communicate with nurses, case managers, families, and others.
