Job summary

Vinfen seeks a dedicated Direct Support professional to assist individuals served in community-based settings. You will help implement treatment support plans and provide compassionate daily living assistance, transportation, and monitoring to promote independence.

The role requires teamwork with an interdisciplinary group, adherence to policies, and ability to support individuals in crisis and community integration. Driving may be required in some cases.

Qualifications

• One to three months experience in a human services setting.
• High school diploma or equivalent is required.
• Driver’s license may be required; ability to pass driving background checks if driving.

Responsibilities

• Develop, implement, and document individual treatment support plans.
• Assist with daily tasks, interventions, scheduling, transportation, and progress monitoring.
• Provide role modeling and support to help individuals integrate into communities.
• Assess physical and emotional status and report changes to supervisor.
• Ensure site safety and assist with daily living tasks and housekeeping.
