Job summary

Qualifications

• At least one year of related experience or training preferred.

• Driving is a requirement for this position with a valid license and vehicle insurance.

Responsibilities

• Assist in developing, implementing, and documenting treatment plans and ISP/IPs.
• Provide training and support to persons served, including daily living skills and safety.
• Assist residents to become connected with their communities and plan activities.
• Provide transportation to appointments, shopping, and community outings.
• Maintain records, logs, and data in the electronic health record and documentation systems.
• Administer and document medications per MAC requirements and policy.
• Coordinate with nurses, case managers, and other staff to ensure continuity of care.
