Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support residents in an ADAPT residential facility. You will promote independence, safety, and a homelike environment while implementing individualized goals and documenting progress.

Responsibilities include daily living assistance, crisis response, medication administration under supervision, and coordinating transport and meals. Strong English communication and NY driver’s license are required; AMAP certification is expected

Qualifications

• High School Diploma or GED required.
• AMAP approval at hire or within six months of date of hire.
• Valid New York State Driver’s License in good standing.
• Ability to read, write, speak and understand English.
• Strong interpersonal and communication skills for working with people we support, families, coworkers, and community partners.
• Ability to respond to crisis situations and make sound decisions under pressure.
• Commitment to confidentiality and safeguarding residents’ rights.
• Ability to work independently and as part of a team.

Responsibilities

• Provides care and supports people we support in achieving greater independence and personal development.
• Ensures safety, health, and well-being of residents at all times.
• Teaches activities of daily living and independence skills.
• Maintains a homelike, respectful atmosphere in the residence.
• Participates in interdisciplinary treatment plans and implements goals.
• Documents daily activities, progress notes, and incident reports accurately.
• Responds to crisis situations and medical emergencies; contacts emergency personnel as needed.
• Administers medications as AMAP under nursing supervision.
• Provides transportation to program, work, appointments, and activities.
• Assists with meal planning, grocery shopping, and meal preparation.
• Participates in housekeeping, laundry, and light maintenance for safety and cleanliness.
• Attends staff meetings and trainings to support ongoing development and quality care.
