Job summary

Qualifications

• High School Diploma is required.
• Experience working with individuals with mental health diagnoses or developmental disabilities is preferred.
• A valid driver's license may be required for transportation duties.

Responsibilities

• Assess and support individuals to achieve personal goals.
• Facilitate relationships and community participation.
• Assist with daily living activities (housekeeping, meals, laundry).
• Ensure rights and safety of individuals are protected.
• Provide input into assessments and planning.
• Complete documentation including progress notes, timesheets, and incident reports.
