Job summary

Volunteers of America Mid-States is seeking a Residential Service Coordinator to oversee services for residents receiving residential habilitation or supported living. You will implement Individual Support Plans and supervise house managers within home and community settings.

The role requires a BA degree or equivalent experience and a Tennessee Driver’s License, with responsibilities spanning case management, resource coordination, and quality assurance.

Qualifications

• BA degree or equivalent experience with the consumer population.
• Current Tennessee Driver’s License.

Responsibilities

• Ensure compliance with DMH/DD Quality Enhancement guidelines for residential supports and services.
• Maintain cleanliness and safety of persons served and their homes.
• Implement and review Individual Support Plans with relevant coordinators and families.
• Coordinate and monitor monthly reviews and share information with medical and day services teams.
• Track consumer outcomes and progress toward each person’s vision.
• Maintain accurate personal records in accordance with guidelines.
• Coordinate staff training and ensure DMH/DD compliance.
• Collaborate with individuals served, guardians, families, and coordinators.
• Participate in management and supervisory planning sessions.
• Maximize benefits for individuals served (e.g., food stamps, housing, utility programs).
• Oversee financial documentation for prudent residential spending.
