Job summary

Qualifications

• Bachelor's Degree in a field eligible for QDDP certification.
• Two years of successful experience in a transferable field.
• One year of supervisory experience required.

Responsibilities

• Coordinate and implement Individual Support Plans and Behavior Support/Medication Plans.
• Serve as liaison between participants, VOA Southeast, families, and case managers to ensure quality services.
• Assist in recruiting, screening, and training staff.
• Ensure staff training is completed as required and attend interdisciplinary team meetings.
• Complete annual assessments, support plans, and monthly/quarterly reports.
• Monitor implementation of all support plans and healthcare services.
• Supervise staff and ensure budgets and expenses stay within guidelines.
• Maintain accurate records and ensure compliance with DMH/MR standards and policies.
