Job summary
Volunteers of America Mid-States in Nashville, TN is seeking a Residential Service Coordinator to oversee housing supports for individuals receiving Residential Habilitation or Supported Living services. The role focuses on implementing Individual Support Plans, coordinating community resources, and supervising house managers to ensure quality services aligned with DMH/DD guidelines and agency mission.

Qualifications
- BA degree OR equivalent experience with consumer population.
- Current Tennessee Driver’s License.
- Experience with residential supports or case management is preferred.
Responsibilities
- Ensure compliance with DMH/DD Quality Enhancement guidelines for residential supports and services.
- Coordinate residential employee training and supervise house managers.
- Implement and monitor Individual Support Plans and conduct monthly reviews.
- Coordinate services with individuals served, guardians, families, and coordinators.
- Monitor records and outcomes to improve client dignity and independence.
