Job summary

Volunteers of America Mid-States is seeking a Home Coordinator to oversee daily operations of supported living homes for adults with IDD. Working under the Residential Service Coordinator, you will lead a team of Direct Support Professionals, ensure quality of care, safety, and compliance with VOA policies.

The role requires at least two years' experience with IDD, knowledge of regulations, and strong supervisory, planning, and communication skills.

Qualifications

• At least 2 years of experience working with adults who have intellectual and developmental disabilities.
• Knowledge of applicable regulatory requirements for supported living.
• Planning and organizational skills
• Skills in good judgement, supervising, and training staff
• Ability to communicate effectively in a courteous and professional manner
• Ability to maintain confidentiality and uphold HIPAA
• Ability to work as a team member

Responsibilities

• Maintain safety and cleanliness of the home, while following VOA policies and procedures
• Supervise and assist consumers in maintaining wholesome and productive lives with opportunities to achieve their goals
• Train Direct Support Staff per mandated curriculum as required by state and VOA policies and regulations
• Accompany consumers on scheduled and emergency medical appointments and ensure communication with the agency’s Registered Nurse
• Attend Interdisciplinary team meetings as assigned by the Residential Service Coordinator
• Complete all weekly, monthly, and quarterly documentation to ensure compliance with PCISP goals and State Regulations
• Train staff on each client’s Individualized Support Plan and ensure annual training is completed
• Plan and coordinate leisure, recreational, and social activities
• Assist consumers in maintaining relationships with family and friends
• Supervise staff and assigned customers; be available on-call
• Develop staff schedules within budgetary constraints and guidelines to provide staff ratios
• Review hours worked and approve timesheets for Direct Support Staff
• Reconcile time off and actual time worked; participate in overtime evaluations
• Monitor and evaluate staff performance
• Recommend disciplinary action for underperformance
• Performance and quality improvement duties as assigned
• Initiate purchase requests to meet consumer needs
• Ensure grocery/supply shopping is appropriate and within budgets
• Maintain vehicle maintenance
• Ensure MARS are accurate and up-to-date
• Communicate health concerns with the RN
• Document medical appointments in the Practical Health System
