Job summary

Cherry Health, Michigan’s largest Federally Qualified Health Center, operates residential re-entry services in Detroit. As Correctional Residential Re-entry Care Coordinator, you oversee daily resident activities, medication administration, and safety within a 24/7 facility under FBOP contract guidelines.

You will orient new residents, document activities, conduct headcounts and searches, and coordinate transportation.

Qualifications

• High School Diploma or GED required.
• Valid MI driver’s license required.
• CPR certification required or must be completed within the first week of employment.
• Associate degree in Human Services or Bachelor’s in Social Work preferred.
• Must be at least 23 years old and have a clean driving record for at least 5 years.
• Experience in mental health or residential correctional programs desirable.

Responsibilities

• Orient and intake all new residents, ensuring rules are understood.
• Maintain detailed documentation of client activities.
• Monitor client activities with headcounts, GPS tracking, and accountability calls.
• Oversee residents’ medication administration and documentation.
• Distribute mail, meals, and transportation passes.
• Transport residents using company vehicles.
• Attend required staff meetings and ensure safety.
• Conduct searches and urinalysis testing as required.
• Maintain cleanliness and safety standards.
