Job summary

Cherry Health in Detroit, MI is seeking a Correctional Residential Re-entry Care Coordinator for 1st shift. This full-time role operates in a 24/7 residential re-entry setting under FBOP contract guidelines, focusing on safety, documentation, and resident support.

You will orient new residents, manage signage in/out, supervise medication administration, and maintain incident reports with de-escalation and professional communication.

Qualifications

• High School Diploma or GED required.
• Valid Michigan driver’s license required.
• CPR certification required or must be completed within first week.
• Associate degree in Human Services preferred.
• Bachelor’s degree in Social Work or related field preferred.

Responsibilities

• Orient and intake new residents, ensuring they are informed of facility rules and expectations.
• Provide guidance and assistance to clients with questions, problems, and concerns.
• Maintain detailed and accurate documentation of client activities and other relevant information.
• Monitor and track client activities on campus and in the community using headcounts, accountability calls, and GPS tracking.
• Ensure all residents sign in and out of the facility and monitor traffic of clients and visitors.
• Conduct random and scheduled searches of both the facility and client belongings.
• Perform urinalysis testing and enforce policies regarding substance use.
• Oversee residents’ medication, ensuring they take it as prescribed, and document the dosage and time taken.
