Job summary

Catholic Charities Worcester County is seeking a Case Manager in Worcester, MA. This non-exempt role provides counseling, develops and evaluates treatment plans for residents of the Crozier House Residential Recovery Home.

You will monitor daily living activities, coordinate referrals, attend meetings, maintain EMR documentation, and advocate for residents' rights while collaborating with Recovery Specialist staff.

Qualifications

• Minimum 3–5 years of experience in dealing with substance use disorder.
• Minimum 3–5 years of experience in dealing with co-occurring disorders as noted in ASAM 3.1 level of care.
• Experience in the use of 12-core functions of the Substance Abuse Counselor.
• Ability to read, write and speak English fluently; Spanish a plus.

Responsibilities

• Provide counseling, both individually and in group settings, and develop, implement, and evaluate treatment planning and educational programs for residents.
• Monitor daily living schedules and residents' well-being within the Crozier House program.
• Collaborate with Recovery Specialist staff and participate in team meetings.
• Complete required EMR documentation in a timely, accurate manner.
• Advocate for residents' rights and coordinate community referrals for services.
