Job summary

Catholic Charities Worcester County seeks a Case Manager in Worcester, MA, to provide counseling and to develop, implement, and evaluate treatment plans for residents at Crozier House Residential Recovery Home Program.

You will supervise daily clinical activities, advocate for residents’ rights, maintain EMR documentation, and collaborate with Recovery Specialist staff. A bachelor’s degree (or CADC/LADC) and 3–5 years in substance use treatment with co-occurring disorders are required.

Qualifications

• 3–5 years of experience with substance use disorder.
• 3–5 years of experience with co-occurring disorders (ASAM 3.1).
• Experience using the 12-core functions of the Substance Abuse Counselor.

Responsibilities

• Provide counseling individually and in groups; develop and evaluate treatment plans for residents.
• Oversee daily clinical activities and monitor residents’ wellbeing.
• Advocate for residents’ rights, benefits, and needs.
• Provide discharge follow-up and community referrals as needed.
• Attend meetings, trainings, and stay current with treatment developments.
• Complete all required documents and EMR entries promptly.
• Collaborate with Recovery Specialist staff and participate in team meetings.
