Job summary

Lutheran Social Service of Minnesota in Minnesota is seeking a Designated Coordinator to oversee residential programs serving 4+ individuals. You will provide leadership, ensure safety, and coordinate service delivery to meet regulatory requirements.

The role requires compliance with Minnesota Statute 245D, strong team supervision, and hands-on support for direct care staff, with an emphasis on quality assurance, budgeting, and collaboration with clinicians and community partners.

Qualifications

• Education pathways required by Minnesota Statute 245D.81, subd. 1–5, for Designated Coordinators.
• Substantial direct care experience in human services fields.
• Flexible availability for emergencies and program needs.

Responsibilities

• Provide guidance, encouragement, support, and safety through relationship building.
• Advocate health and wellness needs and promote independence in essential life skills.
• Monitor medication administration and medical treatments as prescribed.
• Maintain individual plans and financial records per regulation and policy.
• Oversee 4+ residential individuals with meals, supplies, and budgeting.
• Coordinate with analysts, therapists, medical professionals, and community partners.
• Provide a minimum of 16 hours hands-on support and manage staffing and shifts.
• Manage budgets, documentation, and communications with regulatory agencies.

Education

Bachelor’s degree in a human services field + 1 year of full‑time direct care experience with individuals with disabilities or adults age 65+.

Associate degree in a human services field + 2 years of full‑time direct care experience.

Diploma in a human services field from an accredited postsecondary institution + 3 years of full‑time direct care experience.

OR a minimum of 50 hours of education/training in human services and disabilities + 4 years of full‑time direct care experience under qualified supervision.
