The role supports residents across group homes with care, documentation, and adherence to plans, reporting to the Group Home Manager, and delivering consistent, person-centered service.

Qualifications

• High School diploma or equivalent.
• Typing and Windows skills required.
• Valid Driver’s License.

Responsibilities

• Implements ISAs and supports daily routines for residents.
• Performs medication administration as assigned and communicates with nursing staff.
• Participates in program development and completes required training.
• Accurately enters data and maintains progress notes and summaries.
