Job summary

Lifeworks, Inc. in Canton, MA is seeking a Teacher/Counselor (Residential Counselor) to support adults with developmental disabilities in our residential programs. This role focuses on advocacy, education, and empowering individuals to live engaged lives.

You will provide direct care, develop ISPs, coordinate with families and staff, and ensure safety and progress toward ISP goals. A strong background with ID/DD and reliable transportation are essential.

Qualifications

• Experience working with individuals with ID/DD.
• Bachelor's degree or associate degree with two years of experience, or HS diploma and five years in Human Services.
• Pass fingerprint background check, CORI, DPPC and Driving background checks
• Valid Driver's License required
• MAP certification preferred
• Excellent verbal and written communication skills

Responsibilities

• Promotes optimal health for individuals through routine, preventive, and emergency health care.
• Assists with hands-on direct care including showering, toileting, shaving, brushing teeth, and meal-time assistance.
• Ensures residential environments are safe and free from harm.
• Provides education, counseling, and physical assistance to achieve ISP goals in various areas.
• Develops, writes, and presents annual ISPs and semi-annual assessments with staff and directors.
• Provides transportation to individuals as necessary.
• Communicates with families, guardians, and other stakeholders to coordinate programming.
• Performs case management, maintains confidential files, and attends regular meetings
