Lifeworks, Inc. is seeking a Teacher/Counselor (Residential Counselor) to work with individuals with ID/DD in our residential programs. This nonprofit organization emphasizes advocacy, empowerment, and opportunity, offering a supportive environment and competitive benefits.

The role involves direct care, program development, communication with families and staff, and transportation as needed. A related degree or substantial experience in human services is required.

Qualifications

• Experience working with individuals with ID/DD.
• Bachelor's or an associate degree in a related field with two years of experience, or HS diploma with five years of related work experience in Human Services.

Responsibilities

• Promotes optimal health for individuals through routine, preventive, and emergency health care.
• Assists with hands-on direct care (showering, toileting, shaving, brushing teeth, meal-time assistance).
• Ensures residential environments are safe and secure and free from harm.
• Provides education training, counseling, and physical assistance to achieve ISP goals in various areas.
• Develops, writes and presents annual ISPs and semi-annual assessments.
• Provides transportation to individuals as necessary.
• Communicates with families, guardians, funding sources, advocates and others to ensure consistent programming.
