Job summary

Lifeworks is hiring a full-time Assistant Teacher/Counselor (Residential Counselor) in Westwood, MA to work with adults with ID/DD in residential programs. The role emphasizes direct care, ISP development, coordination with families and staff, and ensuring safe, supportive environments.

Candidates should have ID/DD experience, a high school diploma or GED, a valid driver’s license, and MAP certification is preferred.

Qualifications

• Experience working with Individuals with ID/DD.
• Highschool Diploma/GED, experience is preferred in related field.
• Must be able to pass Fingerprint background check, CORI, DPPC and Driving background checks.
• Valid Driver's License required.
• Valid MAP certification is preferred.
• Excellent verbal and written communication skills.

Responsibilities

• Promotes optimal health for individuals through routine, preventive, and emergency health care.
• Assists with hands-on direct care including showering, toileting, shaving, brushing teeth, and meal-time assistance.
• Ensures residential environments are safe and secure and free from harm.
• Provides education, training, counseling, and physical assistance to achieve ISP goals in areas such as functional academics, health and personal hygiene, social and leisure skills, communication, community mobility, self-esteem, and money management.
• Develops, writes and presents annual ISPs and semi-annual assessments with residential staff and directors.
• Provides transportation to individuals as necessary.
• Communicates with family, guardians, and other stakeholders to ensure consistency of programming.
• Performs case management, maintains confidential files, and attends regular meetings.
