Job summary

Agape of Appleton, Inc. is seeking a full-time Residential Case Manager to lead the AFH program. You will provide case management, program management, supervise staff, and deliver direct care support to individuals.

The role includes developing ISPs, promoting community integration, advocating for participants, and guiding staff during challenging behaviors. Weekly interviews occur on Thursdays from 9am–3pm CT.

Qualifications

• Associate degree or Bachelor's in health-care field required.
• Minimum 2 years of management experience preferred.
• Experience in group home setting a plus.

Responsibilities

• Provide inter-intra agency case management.
• Develop, update and implement ISPs and ensure services per agreements.
• Promote community integration and social functioning.
• Provide advocacy to safeguard rights and dignity.
• Guide direct care staff and support during challenging behaviors.
• Provide crisis response as needed.
• Provide leadership, training, coaching and performance management to staff.
• Deliver direct care to participants based on program needs.
• Program currently serves 3–4 participants.
