Job summary

Agape of Appleton, Inc. is seeking a full-time Residential Case Manager to lead our Agape AFH program. The role involves case management, program oversight, staff supervision, and direct care support for individuals. Weekly open interviews are held on Thursdays from 9am to 3pm CT.

The position requires an associate degree (or bachelor’s) in a health-care field, at least 2 years of management experience preferred, and a valid US driver’s license. Visa sponsorship is not offered.

Qualifications

• Associate degree in a health-care related field or Bachelor’s degree in a health-care field.
• At least 2 years of management experience preferred.
• Experience in a group home setting is a plus.
• Must be 21+ years old with a valid driver's license.
• Authorized to work in the United States; visa sponsorship not offered.

Responsibilities

• Provide case management and program management within the Agape AFH program.
• Supervise staff and provide training, coaching and performance feedback.
• Deliver direct care support and crisis response as needed.
• Develop and update Individual Support Plans (ISPs) per contractual agreements.
• Promote community integration and advocate for individual rights.
