Job summary

Agape of Appleton, Inc. is seeking a full-time Residential Case Manager to lead our AFH program. Responsibilities include case management, program oversight, staff supervision, and direct support.

The role also involves crisis response and ensuring services meet contractual agreements. Qualified candidates have an associate or bachelor’s degree in a health-care field, with at least 2 years of management experience preferred. Valid driver’s license required.

Qualifications

• Associate or Bachelor’s degree in health-care field with relevant experience.
• Minimum 2 years of previous management experience preferred.
• Experience in group home setting is a plus.
• Must be at least 21 years old and hold a valid driver’s license.

Responsibilities

• Provide and oversee case management across the program.
• Develop and implement Individual Support Plans (ISPs) and ensure compliant services.
• Lead and train direct care staff; supervise performance and provide coaching.
