Catholic Charities Worcester County is seeking a dedicated Case Manager for the Crozier House Residential Recovery Home Program in Worcester, MA. You will provide counseling, develop and evaluate treatment plans, and coordinate services to support residents’ recovery and well-being.

The role requires 3–5 years in substance use disorder treatment, experience with co-occurring disorders, and proficiency in EMR and Microsoft Office. Strong communication, advocacy, and teamwork skills are essential.

Formación

• Minimum of 3–5 years of experience in dealing with substance use disorder.
• Minimum of 3–5 years of experience in dealing with co-occurring disorders as noted in ASAM 3.1 level of care.
• Experience in the use of 12-core functions of the Substance Abuse Counselor.

Responsabilidades

• Provide individual and group counseling.
• Develop, implement, and evaluate treatment plans and educational programs.
• Advocate for residents' rights, benefits and needs.
• Provide discharge follow-up and community referrals.
• Attend meetings and trainings to stay current with treatment developments.
• Complete all documents, forms and EMR entries accurately and timely.
• Follow Crozier House policies and participate in team meetings.
• Conduct weekly progress meetings with caseload residents.
• Collaborate with Recovery Specialist staff and take leadership in emergencies when needed.
• Admit new residents and manage program intake processes.
• Explain Crozier House programs to community members.
