Job summary

Promises Behavioral Health in Worcester, MA is seeking a passionate Full-Time Case Manager to join our team. The Case Manager will plan, implement, and coordinate comprehensive client services during residency and transition to continuing care, including psychoeducational and motivational instruction.

The role supports one of the trusted behavioral health programs within the Advanced Recovery Systems family and involves maintaining client files, coordinating transportation, and providing ongoing

Qualifications

• Bachelor's degree preferred or equivalent experience.
• Knowledge of case management and substance abuse/mental health services.
• Experience developing rapport with substance-abusing populations and referral/support systems.
• Experience managing patient services and computerized/manual records.
• Experience complying with internal, local, state and federal requirements.

Responsibilities

• Screen, intake, orientation, and assessment.
• Develop and update individualized service plans and referrals; provide/arrange transportation; follow up on referrals.
• Provide at least three individual case management sessions per patient per week; document reviews.
• Assist patients in developing motivation for recovery and early behavior change.
• Maintain patient files and documentation in medical records.
• Coordinate services with agencies, resources, and families; arrange transportation for interviews/continuing care.
