Job summary

The Center for Family and Child Enrichment, Inc. is seeking a Residential Case Manager to support individuals residing in a group home, conducting intake assessments, and developing individualized care plans.

You will coordinate services with healthcare providers, social workers, and community resources, provide direct support and life skills training, and advocate for residents’ rights, education, employment, and housing.

• Knowledge of community resources and support services.

Responsibilities

• Conduct intake assessments and develop individualized care plans.
• Coordinate services with healthcare providers, social workers, and community resources.
• Provide direct support, counseling, and life skills training.
• Advocate for residents’ rights and access to education, employment, and housing.
• Maintain accurate case documentation and monitor resident progress.
• Address mental health and substance use concerns; provide crisis intervention.
• Participate in team meetings and ensure compliance with policies and regulations.
• Perform other duties as assigned.
