Job summary

The Center for Family and Child Enrichment, Inc is hiring a Targeted Case Manager to deliver care coordination and supportive services for children and families in behavioral health and child welfare programs.

Responsibilities include assessing needs, developing service plans, coordinating referrals to healthcare and community resources, and advocating for client access while maintaining HIPAA compliance. Requires a Bachelor's degree and valid driver's license.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Experience in case management, behavioral health, child welfare, or community-based services preferred.
• Knowledge of Medicaid-funded Targeted Case Management services preferred.
• Valid Driver's License, current auto insurance, and access to a reliable vehicle.

Responsibilities

• Provide Targeted Case Management services to children and families in accordance with program and Medicaid requirements.
• Assess client needs and develop, implement, and monitor individualized service plans.
• Coordinate referrals and connect clients and families with healthcare, behavioral health, educational, and community resources.
• Advocate for client needs and support access to appropriate services.
• Maintain regular contact with clients, families, providers, schools, and community agencies.
• Monitor client progress and update service plans as needed.
• Maintain accurate and timely case documentation, progress notes, and required records.
• Collaborate with internal teams, child welfare agencies, schools, and external providers to coordinate services.
• Participate in case staffing, team meetings, quality assurance activities, and other program requirements.
• Protect client confidentiality and comply with HIPAA, Medicaid, TCM, and organizational policies.
• Perform other duties as assigned.
