Job summary

WellLife Network in Jamaica, NY is seeking a dedicated care coordinator to join our nonprofit health and human services team. You will coordinate daily staff assignments, provide comprehensive care, and document consumer data in Avatar, while ensuring safety and confidentiality.

Responsibilities include supervising DSPs, assisting residents with daily living, and participating in interdisciplinary planning. The role offers health benefits, retirement plan, and opportunities for growth.

Qualifications

• High School Diploma required; Associate’s degree preferred or combination of education and experience.
• 1–2 years experience with developmentally disabled population.
• NYS driver's license required; supervisory experience preferred.

Responsibilities

• Coordinate daily staff assignments to support resident care.
• Provide all aspects of care to residents to enable independence and productivity.
• Enter consumer data documentation in Avatar accurately.
• Supervise DSPs, train and orient staff; model professional behavior.
• Assist consumers with personal needs, activities of daily living and self-care.
• Participate in interdisciplinary team processes including planning and monitoring.
• Dispense medication to residents and maintain medical certification.
