Job summary

Qualifications

• Experience identifying habilitative and behavioral needs using formal/informal assessments.
• Ability to design individualized behavior support plans per regulations.
• Familiarity with data collection and monthly reporting processes.

Responsibilities

• Identify habilitative, behavioral and psychiatric needs via assessments.
• Conduct FBAs to identify factors contributing to behavior.
• Develop and implement written behavior support plans.
• Train staff and monitor data collection and plan effectiveness.
• Ensure documentation, consent, and approvals per regulations.
• Provide crisis support and collaborate with leadership.
