Job summary

StoryPoint Group in Kentucky is seeking an Assistant Wellness Director to guide clinical resident care by delivering a 1440 experience, improving performance, and leading staff connections.

Requirements include an LPN license in Kentucky with supervisory experience in Assisted Living/Memory Care, plus solid reading, reporting, and communication skills.

You will lead the team, coordinate care conferences, ensure correct care at the right price, and uphold 1440 standards with compassion.

Qualifications

• LPN license in Kentucky is required and supervisory experience is preferred.
• Ability to read safety rules and write routine reports.
• Strong communication and math skills for care evaluations and pricing.

Responsibilities

• Lead and be accountable for your team and their performance.
• Oversee IL/AL care conferences to ensure right care, at the right price with the right staff.
• Foster Employee First by setting clear expectations and providing regular feedback.
• Oversee resident care and maintain clear, regular family communication.
• Lead to 1440 Care Standards understanding and application.
• Perform quarterly resident admissions/evaluations and set measurable goals.
• Obtain written orders for changes to care plans.
• Verify medications and coordinate with pharmacy for accuracy.
• Review incident reports to determine need for care conferences.
• Schedule health education sessions for residents.
• Communicate emergency policies and provide updated information to staff.
• Strive for continual organizational improvement.
• Be available for a flexible schedule including on-call, weekends, and holidays.
• Perform other duties as assigned
