Responsibilities

  • Organizes transportation of residents to appointments based on priority of needs.
  • Transports residents to and from inpatient/outpatient programs and facilities.
  • Develops driving schedule aligned with resident appointments and program destinations.
  • Reports changes in resident status during transport to clinical supervisor.
  • Maintains safe driving practices and vehicle cleanliness and routine maintenance logs.
  • Assists clinical staff with lifting and transferring as ordered; supports resident care during transport.
  • Communicates with outside facilities and nursing staff for ongoing scheduling and service coordination.
  • Provides adequate hydration to residents and protects resident personal property.
