Job summary

Apploi is seeking a dedicated transportation coordinator to assist in moving residents to medical appointments and discharge-related travel. You will work under licensed nursing supervision, ensuring safety and smooth transitions for each resident.

The role requires a valid driver's license, a state-approved Nurse Aide credential, and current BLS/CPR. You will coordinate schedules, transport records, and liaise with hospitals and clinics to maintain service quality.

Qualifications

• Valid driver's license in the applying state.
• Nurse Aide Training & Competency Evaluation Program; name on state registry.
• Current BLS/CPR certification per state requirements.

Responsibilities

• Organizes transportation of residents based on priority of needs.
• Transports residents to and from inpatient/outpatient programs and facilities.
• Creates driving schedule aligned with resident appointments and destinations.
• Reports resident status changes during transportation to supervisor.
• Transports medical records for signatures as needed.
• Maintains safety and vehicle condition; logs mileage.
• Assists with safe lifting/transfers as directed by medical staff.
• Coordinates with outside facilities to ensure services are scheduled.
• Reports scheduling changes to DON and follows up.
• Provides hydration to residents per daily needs.
• Protects residents' personal property; other duties may be assigned.
