Responsibilities

• Conduct community outreach including educational and listening sessions.
• Build and maintain referral networks to connect residents with resources.
• Assess health care and social services needs through mapping and interviews.
• Coach behavior change and support residents to achieve health goals.
• Travel extensively to community locations and partner agencies.
• Participate in team meetings and contribute to service delivery.
• Educate residents about available services and patient rights.
• Respond to public health emergencies as needed.
