Job summary

South-Middlesex-Opportunity-Council (SMOC) in Framingham, MA is seeking a Case Manager, Resident Support to provide triage case management and referrals to residents in supportive housing. The role includes developing Individualized Service Plans and coordinating services with housing staff to promote self-sufficiency and housing stability.

The position requires strong assessment and advocacy skills, a commitment to serving diverse, low-income populations, and the ability to work independently.

Qualifications

• Bachelor's Degree or CDAC with 3 years of related field experience.
• Minimum one year of assessment, advocacy, and case management experience.
• Sensitivity to low-income people from diverse backgrounds.
• Excellent listening skills.
• Ability to work independently.
• Good written and verbal communication skills.
• Bi-lingual preferred.
• Must have valid driver’s license, reliable transportation and meet insurance standards.

Responsibilities

• Provide triage case management services for residents in assigned properties, including assessment, referrals and service linkages.
• Conduct time-limited follow up and support as clients access new services and action plans.
• Establish an Individualized Service Plan with required individuals as needed.
• Provide information and referrals to community resources (fuel assistance, food stamps, food pantries, etc.).
• Participate in team meetings and service planning with the Opportunity Center team.
• Work with tenants to identify problem areas and address needs in a non-judgmental way.
• Support tenants in financial literacy and planning through workshops and information/referral.
• Maintain up-to-date case notes, logs and referrals; enter data daily.
• Collaborate with Property Managers to coordinate service delivery across properties.
• Assist Property Manager to ensure readiness for new properties.
• Prepare and submit funder and other reports in a timely manner.
• Engage clients by addressing needs inside or outside the scope of work.
• Attend engagement meetings and communicate with clients and staff.
• Maintain confidentiality per laws and funder requirements.
• Ensure compliance with program/department/funder requirements and SMOC policies.
• Other duties as assigned.
