Job summary

Community Human Services Corp. seeks a Community Support Specialist for the Bridge Housing program on Second Avenue. The role involves direct case management, crisis support, and coordination of services for residents in a 24/7 residential setting.

You will maintain documentation, engage in life-skills training, and collaborate with the case management team to support housing and employment goals.

Qualifications

• High school diploma and two years’ case management or related experience.
• Ability to complete required documentation weekly and maintain files.
• Clearances (Act 34/33, FBI) upon hire; CCM and First Aid/CPR/AED certifications desirable.
• Ability to work in a 24/7 residential facility with shift coverage requirements.

Responsibilities

• Develop professional, therapeutic relationships with residents.
• Provide direct case management and assist with benefits and resources.
• Meet residents weekly to review goals and progress.
• Perform safety checks and maintain residence cleanliness and safety.
• Document incidents, progress notes, and facilitate life skills planning.
