Job summary

Community Human Services is seeking a part-time Community Support Specialist for the Bridge Housing program on Stanton Avenue. The CSS provides case management, crisis support, and linkages to benefits, housing and community resources in a 24/7 residential setting.

The role requires strong communication, cultural competence, crisis de-escalation abilities, and the capacity to work across residential sites. Documentation and required training align with Housing First and Harm Reduction models.

Qualifications

• High school diploma plus two years’ case management or related experience.
• Act 34, Act 33, and FBI fingerprint clearances required on hire.
• CCM and First Aid/CPR/AED certifications required or willing to obtain on hire.

Responsibilities

• Develop a professional and therapeutic relationship with residents.
• Provide direct case management to residents and assist with benefits and housing resources.
• Meet weekly with residential caseloads to review goal plans and progress.
• Perform safety and security checks inside and outside the facility as directed.
• Document daily notes, progress reports and goal records in the agency system.
• Coordinate enrollment in life skills workshops and employment services.
