• High school diploma plus two years of case management or related experience.
• CCM and First Aid/CPR/AED certifications required or willing to obtain.
• Clearance checks (Act 34/33 and FBI) required on hire.

Responsibilities

• Develop professional, therapeutic relationships with residents.
• Provide direct case management and assist with benefits access.
• Conduct weekly goal reviews and document progress in records.
• Perform safety checks and maintain facility cleanliness and safety.
• Coordinate life-skills workshops and employment resources with the team.
• Ensure incident reports and progress notes are completed timely.
