Job summary

Community Human Services Corp. seeks a Community Support Specialist for Bridge Housing on Stanton Avenue. The role provides case management and supportive services to residents in CHS Residential Programs, using Housing First and Harm Reduction approaches.

The CSS works across sites, maintains required documentation, and supports life skills development. Requires a high school diploma with two years of case management experience and appropriate clearances.

Qualifications

• A high school diploma plus two years’ work experience in case management or related fields.
• Act 34/33/FBI clearances required; CCM and First Aid/CPR/AED certifications preferred.
• Experience with housing services and crisis intervention is preferred.

Responsibilities

• Develop professional, therapeutic relationships with residents.
• Provide direct case management toward housing goals and benefits access.
• Meet weekly with caseloads to review plans and progress.
• Perform safety checks and maintain unit cleanliness and safety.
• Document progress notes, incidents, and required reports promptly.
• Coordinate with team to enroll residents in life skills workshops and jobs services.
• Provide life skills coaching to enhance self-sufficiency.
• Ensure compliance with program policies and state regulations.
