Qualifications

• High school diploma required.
• Two years of case management or related life/work experience.
• CCM and First Aid/CPR/AED certifications preferred or willingness to obtain.
• Background checks: Act 34, Act 33, FBI clearance required.

Responsibilities

• Develop professional, therapeutic relationships with residents.
• Provide direct case management and assist with benefits and resources.
• Meet weekly with residents to review goal plans and progress.
• Perform safety and security checks as required.
• Maintain and clean common areas and turnover of rooms.
• Distribute meals and ensure safe food handling.
• Collaborate with case management teams to set housing and life-skills goals.
• Coordinate enrollment in life-skills workshops and training sessions.
• Document daily notes, incidents, and progress in the agency database.
