Job summary

PACS is seeking a Licensed Social Worker to join our post-acute care team. The role focuses on assisting residents and their families, developing care plans, and coordinating services to promote the highest practicable level of well-being.

Qualifications include a Bachelor’s Degree in Social Work or Human Services, 2 years of supervised social work in healthcare, MSW preferred. Strong communication and counseling skills are essential.

Qualifications

Responsibilities

• Provide social services to promote physical, mental and psychosocial wellbeing.
• Develop care plans for social services needs with the interdisciplinary team.
• Counsel residents and family members.
• Coordinate discharge planning and arrange outpatient services as ordered.
